Position Overview
Physical Therapist (PT) – Home Care, Kettering, KY. You will provide home‑based therapy care for patients as directed by the attending physician, helping them achieve positive outcomes in their recovery journey.
Schedule & CompensationSchedule:
Saturday, Tuesday, Thursday 8:30am–5:00pm. 8 PIQs a day. Rotational on‑call weekend. 1 holiday per year.
Compensation includes a guaranteed base salary with an uncapped bonus structure, mileage reimbursement, and additional bonus opportunities.
Responsibilities- Plan and execute skilled physical therapy services ordered by the attending physician.
- Observe, record, and report patient reactions, changes, and progress; maintain accurate documentation.
- Perform OASIS assessments and adjust the plan of care as needed.
- Treat patients to relieve pain, develop or restore function, and maintain maximum performance using techniques such as exercise, massage, heat, water, etc.
- Reduce re‑hospitalizations by front‑loading visits for high‑risk patients and teach preventive measures.
- Act as a family educator and advocate, counseling patients and families on disease/injury management and equipment use.
- Supervise PTAs and, when appropriate, nurse assistants; communicate supervisory visit results.
- Adhere to all agency policies, Medicare/Federal guidelines, and regulatory requirements.
- Attend in‑service training and mandatory agency meetings.
- Compassionate communicator with a positive attitude.
- Patience and ability to work with patients, families, physicians, and coworkers.
- Exceptional attention to detail and observant.
- Physical Therapist licensed in the state of employment.
- Minimum one‑year experience in an acute care setting.
- Home‑care experience preferred.
- Must be able to work one dedicated weekend day per week.
- Valid driver’s license and auto insurance in your name.
- Capable of meeting all physical demands of the role.
