How You’ll Work
  • Provide comprehensive skilled services as ordered by the attending physician.
  • Educate patients and their family members and ensure the safety of the patient.
  • Complete accurate documentation and remain compliant with all legal rules and regulations.
Qualifications & Attributes
  • Physical Therapist with a current license in the state of employment.
  • Minimum of one year of experience as a PT in an acute setting is required.
  • Home care experience is desired.
  • Ability to effectively communicate and create positive impressions with patients, families, physicians, and co‑workers.
  • Knowledgeable on therapy best practices and able to make appropriate physical therapy judgments.
  • Detail‑oriented, observant, and disciplined with a strong work ethic, able to prioritize and be timely.
  • Valid driver’s license and auto insurance with your name as a listed driver.
  • Ability to identify a situation and handle it with the best possible solution.
Major Areas of Responsibility
  • Provide skilled physical therapy in a patient’s home as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
  • Perform OASIS assessments to develop an individualized plan of care and make adjustments as needs change.
  • Complete initial evaluation and admission visit within 48‑72 hours of referral.
  • Develop and adjust plan of care, front‑load visits for high‑risk patients, and treat patients to relieve pain, develop or restore function, and maintain maximum performance using exercise, massage, heat, water, etc.
  • Arrange provision of services on an outpatient basis that cannot be given in the patient’s home.
  • Promote continuity of care with appropriate admissions, transfers, and discharges.
  • Counsel patient and family on disease/injury management in all phases of physical therapy.
  • Incorporate patient and family in plan of care development and instruct on proper use of equipment (wheelchairs, braces, walkers, crutches, canes, and other devices).
  • Ensure patient/family know financial responsibility for needed equipment by utilizing DME department and/or MSW.
  • Teach patients preventative measures and decision making to decrease re‑hospitalizations.
  • Listen to patient and family members.
  • Oversee and supervise therapy services provided by PTAs and, when appropriate, nurse assistants.
  • Evaluate plan of care based on patient response and adjust as needed.
  • Communicate results of supervisory visits with PTAs and nurse aides.
