Position: Physical Therapist (PT)

Schedule:

Monday through Friday 8:30 am to 5:00 pm. Territory:
Dayton, West Carrollton, Moraine. Company:

How You’ll Make a Difference

As a Physical Therapist, you will provide compassionate home‑based therapy care as directed by the attending physician, interacting with patients and families throughout their recovery journey.

How You’ll Work

You will deliver skilled services as ordered by the attending physician, educate patients and families, ensure patient safety, complete accurate documentation, and comply with all legal regulations.

Major Areas of Responsibility
  • Plan of Care:
    Provide skilled physical therapy as ordered, observe, record, and report patient reactions and progress; perform OASIS assessments to develop individualized plans and adjust as needed.
  • Patient Outcomes:
    Treat patients to relieve pain, develop or restore function, and maintain maximum performance using exercise, massage, heat, water, etc.
  • Minimize Patient Risk:
    Decrease re‑hospitalizations by front loading visits for high‑risk patients and teaching preventative measures.
  • Family Educator/Advocate:
    Counsel patients and families about disease/injury management; instruct on proper use of equipment (wheelchairs, braces, walkers, crutches, canes, prosthetics, orthotics).
  • Supervision:
    Oversee and supervise therapy services provided by PTAs and, when appropriate, nurse assistants.
  • Communication/

    Collaboration:

    Communicate with PTAs and nurse aides regarding supervisory visit results.
  • Policies:
    Complete all clinical documentation following protocol and Medicare/Federal guidelines.
  • Rules and Regulations:
    Follow agency policies, procedures, rules, and regulations; communicate schedule changes to schedulers or supervisors.
  • Operations:
    Attend in‑service training and mandatory agency meetings.

  • Attention to detail and observant of instructions.
Requirements
  • Physical Therapist licensed in the state of employment.
  • Valid driver’s license and auto insurance in your name.
  • Ability to meet all physical demands required for the role.
