SUMMARY
Physical Therapist Assistant (PTA) provides comprehensive skilled services under the direction of a Physical Therapist. The PTA educates patients and family members, ensures patient safety, completes accurate documentation, and remains compliant with all legal rules and regulations.

- Current Physical Therapist Assistant license in the state of employment.
- Minimum one year of PTA experience in an acute setting.
- Home‑care experience desired.
- Effective communication with patients, families, physicians, and coworkers.
- Knowledgeable in therapy best practices and capable of making appropriate PT judgments.
- Detail‑oriented, observant, disciplined, and able to prioritize and complete tasks timely.
- Valid driver’s license and auto insurance with your name as a listed driver.
- Provide skilled services in patients’ homes according to the Physical Therapist’s plan of care.
- Observe, record, and report patient reactions, changes, and progress.
- Perform physical therapy procedures and diagnostic tests medically prescribed.
- Treat patients to relieve pain, develop or restore function, and maintain maximum performance using exercises, massage, heat, water, etc.
- Assess functional problems and provide interventions, working with the Physical Therapist toward goals.
- Communicate effectively with physicians, therapists, and coworkers.
- Participate in case conferencing and coordinate care.
- Recommend or adjust prosthetic/orthotic devices.
- Educate staff, patients, and families on modalities, safety measures, and supportive activities.
- Instruct on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other devices.
- Listen to patients and family members.
- Foster independence to reduce agency dependency.
- Complete clinical documentation per agency protocol and Medicare/Federal guidelines.
- Conduct chart audits as requested.
- Communicate with scheduler about any changes.
- Participate in on‑call requirements, case conferences, in‑service trainings, mandatory meetings, and orientation/training of new employees.
- Drive to the patient’s primary location per scheduled visit.
- Maintain daily attendance at assigned visit locations.
- Document all aspects of subsequent, discharge, evaluation/recertification/resumption visits within specified time frames.
- Follow the plan of care as permitted within the scope of practice for a Licensed Physical Therapist Assistant, per state regulations.
Travel required. The role demands reading, speaking, writing, computer usage, hand/finger dexterity, vision, hearing, walking, bending, and standing. Frequently includes driving and lifting up to 50 lbs. with or without assistance.
