Work Schedule:
Full‑time, Monday through Friday, 8:30AM to 5:00PM; no weekends or holidays required.
Territory:
Champaign County, Clark County, Miami County, Shelby County, and parts of Logan County (West Liberty, DeGraff, Quincy).
Care and compassion are at the heart of what you’ll do as an Occupational Therapist (OT). You’ll be part of an interdisciplinary team focused on providing compassionate quality care and producing positive outcomes for patients. You’ll provide home‑based therapy care directed by the attending physician, interact with patients’ families, and support patients through every step of their recovery journey.

- Plan of Care: Provide skilled occupational therapy as ordered by the attending physician; observe, record, and report patient reactions, changes, and progress.
- Evaluation: Assess the patient’s level of functioning using diagnostic and prognostic procedures.
- Patient Outcomes: Use activities and home exercise programs to improve range of motion, muscle strength, and coordination.
- Risk Minimization: Reduce rehospitalizations through preventative measures and patient education.
- Family Educator/Advocate: Counsel patients and families on disease/injury management and proper use of adaptive equipment.
- Supervision: Oversee and supervise total care provided by Therapist Assistants.
- Communication/
Collaboration:
Coordinate care with team members and report supervisory visit results. - Policies & Documentation: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Rules & Regulations: Understand and follow agency policies, procedures, and regulatory changes; communicate schedule changes as needed.
- Operations: Attend in‑service training, mandatory agency meetings, and other team events.
- Compassionate communicator with a positive attitude.
- Patience and empathy when working with patients, families, physicians, and coworkers.
- Attention to detail and the ability to stay observant and follow directions.
- Occupational Therapist with a current license in the state of employment.
- Minimum one‑year experience as an OT in an acute care setting (home‑care experience preferred).
- Valid driver’s license and auto insurance under own name.
- Capable of all physical demands of the position.
