The Physical Therapist Assistant (PTA) delivers comprehensive skilled services under the direction of a Physical Therapist. The PTA educates patients and families, ensures patient safety, and completes accurate documentation in compliance with all legal rules and regulations. The company follows the state’s scope of practice for licensed PTAs.

- Physical Therapist Assistant with a current state license.
- Minimum one year of PTA experience in an acute setting; home‑care experience desired.
- Effective communication skills that create positive impressions with patients, families, physicians, and coworkers.
- Knowledgeable in therapy best practices and able to make appropriate physical therapy judgments.
- Problem‑solving skills to identify situations and implement optimal solutions.
- Detail‑oriented and observant.
- Disciplined work ethic with strong prioritization and time‑management skills.
- Valid driver’s license with auto insurance and name listed as driver.
- Provide skilled services at patients’ homes per the Physical Therapist’s plan of care.
- Observe, record, and report patient reactions, changes, and progress; perform physical therapy procedures and diagnostic tests as medically prescribed.
- Implement interventions to relieve pain, restore function, and maintain maximum performance using modalities such as exercise, massage, heat, and water.
- Assess functional problems and collaborate with the Physical Therapist to achieve functional improvement goals.
- Communicate effectively with physicians, therapists, and coworkers.
- Participate in case conferences to promote coordinated care.
- Recommend, evaluate, or adjust prosthetic/orthotic devices as appropriate.
- Educate staff on specific physical therapy modalities when applicable.
- Educate patients and families on therapy techniques, safety measures, and supportive activities.
- Instruct patients and families on proper use of equipment such as wheelchairs, braces, walkers, crutches, and canes.
- Promote independence to reduce reliance on the agency.
- Complete clinical documentation per agency protocol and Medicare/Federal guidelines.
- Document all aspects of treatment, assessments, and patient education.
- Conduct chart audits as requested at agency locations.
- Notify schedulers of any changes as needed.
- Participate in on‑call duties and case conferences.
- Attend in‑service training and mandatory agency meetings.
- Assist in orientation and training of new employees.
- Perform other duties as assigned.
- Drive to patients’ primary locations per scheduled visits.
- Attend all assigned visit locations daily.
- Document all aspects of subsequent, discharge, evaluation/recertification, and resumption visits within 24 hours.
- Document all aspects of start‑of‑care visits within 48 hours.
- Complete and submit required documentation within company deadlines.
- Follow the plan of care within the scope of practice for a Licensed PTA per the state of employment.
