• Provide skilled physical therapy services in patients’ homes.
• Complete evaluations and develop individualized plans of care.
• Help patients improve mobility, strength, safety, and independence.
• Educate patients and families on therapy goals, equipment use, and injury/disease management.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians, PTAs, nurses, and the interdisciplinary care team.
• Complete documentation in accordance with agency and Medicare guidelines.
• Support continuity of care through admissions, transfers, and discharges.

• Current Physical Therapist license in the state of employment.
• Minimum of 1 year of PT experience in an acute care setting required.
• Home health experience preferred.
• Valid driver’s license and auto insurance.
• Strong communication, time management, and clinical judgment skills.
• Comfortable working independently while staying connected to a supportive team.
