Job Overview

Occupational Therapist (OT) – Home Care.

Schedule:

Monday‑Friday 8:15am – 5pm, no on‑call or holiday rotation. Territory:
Cedarville, Jamestown, Wilberforce, Xenia. Agency:
Kettering Home Care.

• Provide skilled occupational therapy services following physician orders.
• Develop, implement, and adjust individualized OT plans of care.
• Observe, record, and report patient progress to physicians and agency staff.
• Evaluate functional status using diagnostic and prognostic procedures.
• Teach patients and families skills and activities to improve mobility and daily living abilities.
• Educate patients and families on safety, equipment use, and disease/injury management.
• Supervise and coordinate care provided by OT Assistants (COTAs).
• Maintain accurate documentation and comply with all clinical, agency, and federal guidelines.
• Attend mandatory in‑service training and agency meetings.

• Current occupational therapist license in the state of employment.
• Minimum one‑year experience in an acute care setting; home‑care experience preferred.
• Valid driver’s license and personal auto insurance.
• Physical ability to meet job demands.
