Alternate Solutions Health Network, LLC's Kettering Home Care is seeking a Community Care Coordinator to coordinate patient care transitions from hospital to home, participate in discharge planning, and ensure smooth handoffs within the health system.
The ideal candidate will hold an Ohio clinical license (RN/LPN/PT/PTA/LSW), have discharge experience, a valid driver’s license, and be able to work Monday–Friday 8:30am–5:00pm with a strong emphasis on compassionate, patient-centered care.
