We are seeking a highly skilled, experienced, accountable, and self-motivated home health LPN or RN to join our agency as a Part-Time Private Duty Nurse (PDN) to care for a client in Kettering / Dayton, OH (Zip Code: 45409), with the option to add additional clients in surrounding areas for additional Part-Time hours.
Immediate Client Needs
- The Dayton client (45409) requires immediate care on Mondays (every week) for 8 hours during the day. There is flexibility on the exact schedule within an 8:00–11:00 a.m. start time and 4:00–7:00 p.m. end time.
- The patient has a loving family involved in care, as well as an evening/overnight PDN.
- LPN or RN is required, with at least 1 year in home health and 1 year of nursing experience. Additional home health experience is highly desired.
- Client is dependent on care, and staff will need to assist with basic patient handling tasks (e.g., pushing, pulling, rolling).
Role Details and Perks
- Immediate Start: 8 hours on Mondays, weekly.
- If you desire additional regular hours, we will seek to secure additional clients near your home (not required). When applying, please indicate the minimum number of hours you require to start (8 or greater).

- No on-call requirements, no weekend requirements (unless desired), and no overnight travel.
- Mileage reimbursement for travel between multiple clients, if applicable.

Starting Pay Rate
- LPNs: $28–$30 per hour
- RNs: $35–$40 per hour
Compensation is based on employee qualifications and certifications, experience, skill specializations, positive employment history, and overall client & team fit.
Qualifications
- Graduate of an accredited school of professional nursing.
- Active LPN or RN license to practice in the state of Ohio is required.
- At least 1 year in home health and 1 year of nursing experience. Additional home health experience is highly desired.
- Active CPR and First Aid certificates are highly desired; recertification will be provided if required.
- Good interpersonal communication skills (written and verbal).
- Desirable character traits: reliable, honest, professional, compassionate, detail-oriented, accountable, friendly, accommodating, respectful.
- Current driver’s license or state reliable transportation is required.
- Excellent attendance record with past employers.
- Ability to meet all basic job requirements.
- Willingness and ability to pass state and federal background check.
Position Summary
A successful PDN at Home Care Network, Inc. performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of the professional organization and agency. All functions are performed per established policies and the Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.

Essential Functions / Areas of Accountability
- Provides professional nursing care under the direction of a Registered Nurse and communicates status/condition changes to the RN per agency guidelines.
- Provides skilled nursing visits as directed by the RN Case Manager and follows the care plan established by the RN.
