Weekend Registered Nurse; RN

Job Overview

As a Registered Nurse (RN) at Kettering Home Care, you will provide one‑on‑one patient care in the patients' homes, working at the top of your license and leading coordinated care for patients under physician direction.

You will provide skilled nursing care in patients' homes as directed by the attending physician, working autonomously and efficiently managing your time while performing all duties within the scope of practice outlined by the state.

• Plan of Care:
  Observe and monitor patient conditions, perform OASIS assessments (we will train you if needed), develop an individualized care plan, and adjust it as required.
• Minimize Patient Risk:
  Administer medication as prescribed by the physician and help decrease re‑hospitalizations by prioritizing visits for high‑risk patients.
• Continuity of Care:
  Manage multidisciplinary care as needed while ensuring continuity through appropriate admissions, transfers, and discharges.
• Family Educator/Advocate:
  Educate the patient and family about disease processes using teach‑back methods to ensure understanding.
• Policies:
  Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations:
  Understand and follow agency policies, procedures, rules, and regulations, and communicate changes in schedule or availability to schedulers or supervisors.
• Operations:
  Attend in‑service training and mandatory agency meetings.

Attention to detail is critical, as is being observant and following directions.

• Registered Nurse with current license in the state of employment.
• Minimum one‑year experience as an RN in an acute care setting.
• Home care experience preferred.
• Valid driver’s license and auto insurance in name as a driver.
• Capable of all physical demands.
