Licensed Practical Nurse; LPN - Weekend

Position Overview

Licensed Practical Nurse (LPN) providing home‑based skilled nursing care for patients in Preble, Montgomery and Greene Counties.

• Plan of Care:
  Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions, notifying the case manager of any changes.
• Minimize Patient Risk:
  Administer medication as prescribed, educate patients on preventative measures, and support reduced rehospitalizations.
• Family Educator/Advocate:
  Educate patient and family on disease process using teach‑back methods.
• Documentation:
  Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Compliance:
  Understand and follow agency policies, procedures, rules, and regulations; communicate schedule changes.
• Operations:
  Attend in‑service training and mandatory agency meetings.

• Compassionate communicator with a positive attitude.
• Patience when working with patients, families, physicians, and coworkers.
• Attention to detail and observant, following directions.

• Licensed Practical Nurse with current license in the state of employment.
• Minimum one‑year experience as an LPN in an acute care setting; home‑care experience preferred.
• Valid driver’s license and auto insurance in your name as a driver.
• Capable of all required physical demands.
