Licensed Practical Nurse; LPN - Weekends

Position Overview

Licensed Practical Nurse (LPN) position with Kettering Home Care.

Schedule:

Saturday, Tuesday, Thursday, 8:15 AM – 5:00 PM.

• Provide skilled nursing care in patients’ homes as directed by the attending physician.
• Plan of Care:
  Follow physician orders, monitor patient conditions and notify case manager of changes.
• Minimize Patient Risk:
  Administer medication, educate patients on preventive measures, reduce rehospitalizations.
• Family Educator/Advocate:
  Teach disease process to patients and families using teach‑back methods.
• Documentation:
  Complete clinical documentation per agency protocol and Medicare/Federal guidelines.
• Compliance:
  Follow agency policies, procedures, rules, and regulations; communicate schedule changes.
• Operations:
  Attend in‑service training and mandatory agency meetings.

• Licensed Practical Nurse (LPN) with current license in the state of employment.
• Minimum one‑year experience as an LPN in an acute care setting; home care experience preferred.
• Valid driver’s license and auto insurance in your name.
• Capable of all physical demands of the role.
