Position: Licensed Practical Nurse I

Qualifications & Attributes
  • Licensed Practical Nurse with current license in the state of employment
  • Minimum one year of experience as an LPN in an acute care setting (two years preferred)
  • Home care experience desired
  • Effective communication and ability to build positive impressions with patients, families, physicians, and coworkers
  • Calm, patient, accommodating, compassionate, and caring
  • Knowledgeable on nursing best practices and able to make appropriate judgments
  • Detail‑oriented, observant, disciplined work ethic, able to prioritize and be timely
  • Ability to follow directions and work as a team member
  • Valid driver’s license and auto insurance with name as listed driver
Major Areas of Responsibility
  • Provide skilled nursing care in the patient’s home as ordered by the attending physician
  • Perform care following order sets and goals made by the RN
  • Observe and monitor patient conditions and notify RNCM of changes
  • Administer medication as prescribed and counsel patient and family on disease/injury processes and management
  • Incorporate patient and family in development of care plan and help reduce readmissions by teaching preventative measures and good decision‑making
  • Listen to patient and family members, complete all clinical documentation per agency protocol and Medicare/Federal guidelines, and document all aspects of treatment, assessments, and education
  • Maintain active LPN license in the state of employment and communicate with scheduler about changes
  • Participate in on‑call requirements, case conferences, in‑service trainings, and mandatory agency meetings
  • Maintain a professional appearance and understand and practice agency policies and procedures
  • Perform any other duties assigned
Essential Functions
  • Drive to patient’s primary location per scheduled visit and attend daily assigned visit locations
  • Document all aspects of visits within 24hours and submit all required documentation within company specifications
  • Provide skilled care within the scope of practice for an LPN and fulfill visits in assigned geographic location per patient need within federal and state specifications
Physical Demands
  • Reading, speaking, writing English; computer/PDA usage; hand/finger dexterity; hearing and vision for close work – frequent (34–66%)
  • Walking, sitting, driving, talking on the phone – constant (66–100%)
  • Bending, standing, lifting up to 50lbs, stretching/reaching, climbing, stooping – occasional (2–33%)
  • Distinguish smell/taste – rare (≤1%)
