• Licensed Practical Nurse with an active license in the state of employment.
• Minimum 1 year of experience as an LPN in an acute care setting (2 years preferred). Home care experience desired.
• Strong communication skills and ability to create positive impressions with patients, families, physicians, and coworkers.
• Calm, patient, and accommodating demeanor.
• Compassionate and caring when working with patients.
• Knowledgeable of nursing best practices and able to make appropriate nursing judgments.
• Ability to identify situations and handle them with the best solution.
• Detail‑oriented and observant.
• Disciplined work ethic with ability to prioritize and be timely.
• Ability to follow directions and work as a team member.
• Valid driver’s license and auto insurance with your name as a listed driver.
• Maintains active LPN license in the state of employment.

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Performs care following order sets and goals made by RN.
• Observes and monitors patient conditions.
• Notifies RNCM of any changes in patient condition.
• Administers medication as prescribed by the physician.
• Counsels patient and family on disease/injury processes and management.
• Incorporates patient and family in development of plan of care.
• Helps decrease re‑hospitalizations by teaching patients preventative measures and promoting good decisions.
• Listens to patient and family members.
• Completes all clinical documentation per agency protocol and Medicare/Federal guidelines.
• Communicates with scheduler for any changes outside normal schedule.
• Participates in on‑call requirements and case conferences.
• Attends in‑service trainings and mandatory agency meetings.
• Maintains a professional appearance as a representative of the company.
• Understands and practices agency policies and procedures.

• Drives to patient’s primary location per scheduled visit.
• Daily attendance at assigned visit locations.
• Documents all aspects of subsequent visits within 24 hours to meet Medicare guidelines.
• Completes and submits all required documentation within specified company requirements.
• Fulfills LPN visits within scope of practice for a Licensed Practical Nurse.
• Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
• Adheres to Medicare guidelines for documentation and timing.
