Position: RN Case Manager - Home Health Agency (PRN or Part-Time: Weekdays, Daytime)

Overview

Job Title:

Home Health RN Case Manager & Recertification Nurse (RNCM). This is a regional leadership role within our agency requiring travel as needed throughout Dayton and the Metro-Dayton areas where clients and caregivers reside. The RNCM reports to the SW OH Regional Program Administrator (local) and/or the Director of Clinical Administration & Operations (corporate). This position is available as PRN or Part-Time, with potential for Full-Time in the future but not guaranteed.

SERVICE AREA
:
Dayton / Dayton Metro and surrounding areas up to a 60-minute radius in Southwest Ohio. Primary/Centralized service areas include Dayton / Dayton Metro area;
Middletown, Springfield, Xenia, Piqua, Urbana and surrounding areas. Travel up to 2 hours daily is desirable but not required; exceptions apply for locations listed as secondary service areas.

Employment & Schedule Type
:
Long-term employment for PRN or Part-Time RNCM/Recert Nurse. This role requires weekday availability (Monday – Friday) between 8am – 5pm, with occasional/rare evening availability scheduled in advance, and consistent, reliable availability for the chosen schedule type.

Responsibilities
  • Case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services.
  • New hire orientations, in-service and supervisory reviews, and input to influence training, care practices, and outcomes for clients, employees and the business.
  • Back-up and occasional skilled nursing visits and services including med sets and dressing changes.
  • Coordinate and document patient status and changes in condition; ensure effective communication among physicians, agency staff, service administrators, other caregivers and clinicians, and the client and family.
  • Travel to multiple counties; no overnight shifts or overnight travel required.
Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year of experience as a Registered Nurse with an active RN license in Ohio; minimum 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aid certifications required or obtainable prior to hire date; experience with OASIS and NON-OASIS assessments required.
  • Strong nursing background, patient care, confidentiality, and documentation skills; supervisory or leadership experience in field staff is a plus.
  • Desirable but not required: nurse supervision experience, people management, peer/staff education and development; ability to build relationships with client payors and case manager entities (e.g., AAA, DoDD County Board, VA) in SW Ohio.
  • Willingness to travel within stated service area; valid driver’s license and reliable transportation; US work authorization.
  • Consent and ability to pass state and federal background checks (including fingerprints).
Reports To

Regional Program Administrator, or Clinical Administrator/Director of Nursing.

Compensation & Schedule Details
  • Compensation: PRN and Part-Time hourly wage (+ mileage) starts between $35–$40 per hour, based on travel radius and coverage.
  • Pay:
    Weekly direct deposit on Fridays.
  • Mileage:
    Round-trip/door-to-door mileage reimbursed.
  • Travel:
    Requires travel within pre-determined service area (minimum 1-hour travel radius); willingness to travel up to ~2-hour daily radius is highly desirable.
  • Contractors will also be considered.
  • No on-call shifts; no weekends required. Weekend services are optional when available but not required.
