What You'll DoProvide skilled nursing visits in patients' homes

Administer medications and treatments

Monitor patient conditions and communicate changes

Educate patients and families on disease management

Complete clinical documentation

Collaborate with physicians, RNs, and the care team

Qualifications

Current LPN license1+ year of acute care experience

Home health experience preferred

Valid driver's license and auto insurance
