Signify Health, part of CVS Health, seeks a Per diem Clinician (NP or PA) to provide in-home health evaluations in Hawaii. The primary workplace is the member's home, with flexibility to travel locally and within licensed states as needed.
Visits last about 45 minutes and include medication/history review, physical examination, and optional tests per the health plan. You will use an iPad workflow; prescribing or lab orders are not required.
