We’re looking for a Licensed Ind Practitioner to consult with physicians, patients, hospital staff, family members, and others to resolve barriers to treatment progress, maintain records of behavioral health interventions, and coordinate referrals to community agencies.

• Consults with physicians, patients, hospital staff, family members, or significant others to resolve barriers to treatment progress and ensure continuity of care.
• Maintains records of behavioral health interventions, assessment, diagnostic impression, and treatment for accurate statistics.
• Makes timely referrals to Federal, state, county, and community agencies.
• Develops relationships with agencies to ensure appropriate service delivery to patients.
• Assesses patients for behavioral health needs and provides referrals as needed.

Must have a Master’s degree and a valid professional license:

• Master's (Required)
• Clinical Social Worker License (LCSW) – EV Accredited Issuing Body
• Licensed Marriage and Family Therapist (LMFT) – EV Accredited Issuing Body
• Licensed Professional Counselor (LPC) – EV Accredited Issuing Body
