Performs moderately complex (journey‑level) case work. Serves as a liaison between clients, their families, and other agencies to obtain assistance and rehabilitative treatment. Works under general supervision with limited latitude for initiative and independent judgment.
Essential Duties and Responsibilities- Interview clients, families, law enforcement officials, schools and health and social welfare institutions to obtain background information.
- Identify psychosocial, economic, and physical needs of clients.
- Assess clients’ support systems and available community resources, schools, and facilities.
- Provide case management services during client admission, discharge, or furlough.
- Counsel clients and families and coordinate placements with community facilities and schools.
- Provide psychosocial rehabilitation skills training, medication training and support, crisis intervention, supported housing, supported employment and other related services.
- Serve as liaison with state and judicial systems and treatment providers.
- Document case records, assessments, recovery plans and progress notes per Center requirements and Texas Administrative Codes.
- Assist in developing individual treatment plans and monitor observance of clients’ rights.
- Participate as needed in individual, group, or family therapy sessions.
- Participate as an active member of an interdisciplinary team.
- Meet and maintain all minimum service requirements per Center and Texas Administrative Codes.
- Maintain department productivity, contract outcome, and documentation requirements.
- Participate in training and time studies for Medicaid Administrative Claiming (MAC).
- Perform related work as assigned.
- Demonstrate high ethical standards, trustworthiness, personal integrity, and strict attendance at 8‑5 work day (or agreed hours) 100% of the time.
- Complete required training and continuing education to maintain competency.
- Drive frequently within the catchment area.
- Experience in a field related to the agency program.
- Graduation from an accredited four‑year college or university with relevant coursework.
- Experience and education may not be substituted for one another.
- Knowledge of trauma‑informed care principles and cultural sensitivity.
- Knowledge of mental illness, rehabilitation methods, casework principles, community resources, and social work practices.
- Ability to assess client needs, coordinate services, and work well with others.
- Proficiency in Microsoft Word and Excel.
- Ability to handle potentially aggressive behavior positively.
- Ability to read, interpret documents and data.
- Ability to communicate effectively, both orally and in writing.
- Use of technology to accomplish assigned tasks.
- Ability to see, hear and talk effectively within job requirements.
