Role OverviewAs the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes. You will record clinical and progress notes, and coordinate other patient care activities and services to deliver the highest quality of care in the geographic service area assigned. You will work under the direct supervision of the Registered Nurse and/or designated supervisor.
PrimaryResponsibilities
- Provide high‑quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other members of the health care team, following all applicable infection control procedures.
- Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency’s electronic visit verification program (as applicable).
- Actively participate in the agency’s care coordination process, including timely reporting of patients’ needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, case coordinators, and community case managers (as applicable).
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current CPR certification.
- Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation.
- Ability to function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
- Ability to work flexible hours as required to meet identified client needs.
- Ability to manage multiple tasks simultaneously.
- Ability to work independently.
- Good communication, writing, and organizational skills.
Pay for this role will range from $24.10 to $36.15 per hour based on full‑time employment. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401(k) contribution (subject to eligibility requirements).
