What Will You Do?

· Provide in-person primary care visits as part of a comprehensive longitudinal care model, working in close collaboration with a virtual PCP who serves as the patient's primary care provider; virtual care modalities may be used as appropriate

· Conduct adult and pediatric wellness visits that address quality care gaps and establish a comprehensive baseline physical exam for all patients

· Provide acute and chronic care requiring physical examination or diagnostic testing, including transition of care visits after high-risk hospitalizations, as needed

· Reconcile and prescribe medications, order and administer labs and diagnostics, and refer to specialist care, as needed

· Partner with an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists and medical assistants upskilled to support social services navigation

· Document care and maintain clear, comprehensive care plans addressing patient and family needs

· Help families identify and access health and social care resources, including food, housing, transportation, and financial assistance

· Participate in community outreach efforts, as needed

· Graduate of an accredited Nurse Practitioner or Physician Assistant program with an active, unrestricted Arizona license

· At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions (preferred)

· Proven skills, knowledge base, and judgment necessary for independent clinical decision-making and problem-solving

· Hands-on clinical skills, including venipuncture, patient assessment, and chronic care support, or willingness to learn

· Ability to work at least 1-2 days per week (8-10 hours per day), with the opportunity to expand hours as the program grows

· Current BLS/CPR certification

· Passion for serving underserved and vulnerable populations with empathy and respect

· Leadership experience in an interdisciplinary care team preferred

· Startup experience preferred

This is a field-based role, providing in-person care at community facilities or in patient homes across the Kingman service area, transporting health care supplies (up to 25 lbs). An unrestricted driver's license, reliable personal vehicle, and auto insurance are required (mileage reimbursed). Candidates must be willing to follow the Nest's Motor Vehicle Safety Policy.
