White Oak Kings Mountain is a 154-bed Long-Term Care, Skilled Nursing, and Short-Term Rehabilitation Facility located at:716 Sipes Street Kings Mountain, NC 28086
White Oak Kings Mountain is seeking a highly organized, detail-oriented, and compassionate RN MDS Coordinator / Nurse Assessment Coordinator (NAC) to join our exceptional this vital role, you will assess residents' needs and strengths through the federally and state-approved MDS assessment process. The Nurse Assessment Coordinator develops individualized, functional Plans of Care that address each resident's physical, emotional, and psychosocial needs while supporting quality outcomes and regulatory compliance.
Position Highlights Full-Time Position Monday–Friday Schedule 8-Hour Day Shifts $15,000 Sign-On Bonus Paid in installments throughout the first year of employment
Responsibilities
Coordinate and complete MDS assessments in accordance with federal and state regulations
Accurately assess resident needs, strengths, and clinical status
Develop and maintain individualized Plans of Care Collaborate with interdisciplinary team members to ensure quality resident outcomes
Monitor assessment schedules and ensure timely submissions
Assist with care planning and quality improvement initiatives
Maintain compliance with CMS, state, and facility requirements
Support reimbursement optimization through accurate documentation and assessment practices
Qualifications
Current, active Registered Nurse (RN) license in North Carolina
Experience with MDS assessments and care planning preferred
Strong organizational and time management skills
Excellent communication and teamwork abilities
Knowledge of Medicare, Medicaid, and long-term care regulations preferred
Ability to work independently while collaborating effectively with an interdisciplinary team
