A rewarding per diem opportunity is available for a skilled and compassionate Nurse Practitioner to deliver in-home health risk assessments throughout Camden County. This flexible role empowers you to create your own schedule, with shifts available Monday through Sunday, and requires only a minimum weekly or bi-weekly hourly commitment.

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members.
• Evaluate patients prior and current medical history, chronic and acute medical conditions, and medication histories.
• Deliver clear education regarding health conditions and medication management.
• Recognize care gaps and provide thoughtful recommendations to promote optimal health outcomes.
• Assessment visits may varyfrom annual evaluations to post-discharge assessments without the need to create treatment plans or prescribe medication.

• Active Nurse Practitioner license issued by the state of Georgia.
• Current CPR certification (BLS required).
• National Board Certification through AANP or ANCC.
• Master of Science in Nursing (MSN) from an accredited program.
• At least 6 months of recent nurse practitioner experience preferred.
• Must be comfortable working independently in a home health environment.
