Role DescriptionWe are seeking a Physician (MD/DO) to join our team caring for patients across three skilled nursing and long-term care facilities in Kingsport, Greeneville, and Rogersville, Tennessee.
This role offers the opportunity to provide meaningful, relationship-based care while maintaining a balanced and sustainable schedule.
Key Responsibilities- Serve as the primary care provider for patients in skilled nursing and long-term care settings, ensuring high-quality, patient-centered care.
- Perform comprehensive patient assessments and evaluations to develop individualized treatment plans aligned with each patient’s goals.
- Diagnose and manage acute and chronic conditions
, coordinating timely interventions to promote recovery and prevent unnecessary hospitalizations. - Document all patient encounters and care plans accurately in medical records, including appropriate ICD-10 coding to support continuity of care and compliance.
- Collaborate closely with nursing staff, facility teams, and other healthcare professionals to deliver coordinated and compassionate care.
- Educate patients and families about health conditions, treatment options, and preventive care to support informed decision-making.
- Complete Medicare recertification visits and documentation to ensure uninterrupted patient services and regulatory compliance.
- Experience in post-acute or long-term care (PA/LTC) preferred
- Physicians with hospitalist, inpatient, or primary care experience are also well-suited
- Passion for improving patient outcomes and managing complex cases
- Strong commitment to compassionate, patient-centered care
- Excellent earning potential: Top candidates can earn $280,000+ annually
