Description
The Program Lead, under the supervision of the Regional Program Director or Assistant Director, assists in oversight of the program and ensures the client‑centered care and case coordination and/or resource linkage to clients is done in a competent, responsible, and ethical manner.
SummaryThe Program Lead supports client‐centered care, collaborates with care coordinators, ensures compliance with Clarvida policy and relevant regulations, and contributes to continuous quality improvement.
Essential Duties And Responsibilities- Assess clients on intake as needed, complete on‑call duties, and ensure Clarvida‑approved behavioral health therapeutic services are being provided to clients and families, including coping skills, behavior modification, parenting skills, and crisis intervention. Lead will ensure that internal and external collaborations occur with behavioral health/medical providers.
- Work with care coordinators to ensure that all assigned clients’ treatment teams collaborate to ensure quality care. Ensure care plans and documentation are consistent with Clarvida policy, state licensing regulations, and COA accreditation requirements.
- Ensure that all clinical documentation has been completed within established time frames and rectify deficits within the allotted period; monitor monthly and quarterly reports.
- Monitor and work with care coordinators to close HEDIS care gaps for clients that have open gaps, reviewing progress weekly.
- Conduct weekly supervision of care coordinators according to contract specifications and COA requirements.
- Facilitate treatment team meetings and review presentations on each assigned client. Attend all quarterly director meetings and regional leadership meetings.
- Support fellow team members to ensure the best outcomes for all Clarvida clients.
- Contribute to the performance improvement cycle by collecting and using data to improve client and program services, complete PQI activities accurately and timely, and participate in MCO evaluations.
- Assist leadership in overseeing client census and productivity standards for care coordinators.
- Help develop program initiatives, network, and collaborate across program areas.
Bachelor’s degree in a Human Service discipline from an accredited four‑year college or university and at least 3 years of relevant experience in the mental health field.
Preferred Education And ExperienceExperience working with children/adolescents in a therapeutic, community‑based treatment environment.
Certificates, Licenses, RegistrationsLicensed driver with automobile insurance in good standing and reliable transportation.
