Adoration Home Health and Hospice is seeking a passionate, dedicated Home Health Clinical Manager to join our team in Kingsport, TN. In this leadership role, you will oversee high-quality, patient-centered care while supporting and mentoring a team of clinicians.

Schedule:

Full-Time, Mon-Fri 8AM-5PM.

• As a Home Health Clinical Manager, you will conduct/participate in quarterly quality improvement meetings, review data, identify trends, and oversee improvement plans.
• Collaborate with the Branch Director in quality improvement activities and corrective actions.
• Oversee responses to external and internal surveys and audits related to quality of care and clinically related KPIs in collaboration with operations leadership.
• Complete documentation and chart reviews to identify variances in standardized care and opportunities for improved performance.
• Hold case conferences to ensure oversight of care and coordination of services with standards being met.
• Facilitate development of performance improvement action plans based on data (e.g., survey results, QAPI, clinical record reviews); communicate expectations and manage performance.
• Ensure staff are evaluated fairly and timely in accordance with company procedures.

• Bachelor s Degree preferred in Nursing from an accredited school of nursing.
• Licensed Registered Nurse (RN) in good standing and currently licensed by the State.
• Less than two years of clinical experience, and at least one year in a related management/supervisory role in home health care.
• Current CPR certification.
• Must meet all agency pre-employment requirements as required by the company and/or State regulations.
• Ability to use company documentation systems and communicate effectively with all levels of personnel.
• Ability to work independently as well as part of a team; capable of handling highly confidential information.
