Position Summary

The Registered Nurse plans, organizes, directs, and coordinates home care services for patients in the Kingsport, TN area.

Responsibilities
  • Develop and initiate the plan of care, and oversee the interdisciplinary assessment and development of individualized plans with patients, families, and caregivers.
  • Initiate preventive and rehabilitative nursing procedures and provide services as ordered by the physician.
  • Provide patient, family, and caregiver counseling and education.
  • Prepare and update clinical notes, and communicate with the home health physician and other health practitioners as needed.
  • Identify and implement discharge planning needs as part of the care plan.
  • Communicate with community health‑related persons to coordinate the care plan.
  • Participate in the agency’s quality assessment, performance improvement program, and agency‑sponsored in‑service trainings.
  • Participate in on‑call duties as defined by agency policy.
  • Ensure arrangements for equipment and other necessary items and services are available.
  • Instruct, supervise, and evaluate Home Health Aide care provided every two weeks.
  • Supervise Licensed Practical Nurses, Licensed Vocational Nurses, Home Health Aides, and Certified Nursing Assistants as directed.
  • Perform other appropriate services and special projects as assigned.
Qualifications & Requirements
  • Current Registered Nurse (RN) licensure in the state of practice.
  • Minimum two years of Registered Nurse experience; home health experience preferred.
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and reliable transportation.
Preferred Knowledge & Skills
  • Excellent time‑management and problem‑solving skills.
  • Outstanding verbal and written communication skills.
  • Self‑motivated and able to work independently while collaborating as part of an interdisciplinary team.
Supervisory Responsibilities

May supervise Home Health Aides, Certified Nursing Assistants, Licensed Practical Nurses, and Licensed Vocational Nurses as needed.

Physical Demands & Work Environment
  • Heavy use of office equipment (computer, tablet, phone).
  • Ability to stand, walk, sit, use hands, reach, balance, see, talk, hear, and drive.
  • Ability to lift over thirty pounds.
  • Significant portion (>50%) of assignments may require travel to client locations.
  • Work schedule may include evening, night, holiday, or weekend assignments.
