Overview

Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in home settings, under the supervision of a Registered Nurse or designated supervisor.

• Provide high‑quality clinical services within the scope of practice defined by state nursing laws, in accordance with the care plan and in collaboration with the health‑care team.
• Document patient visits thoroughly in the electronic medical record, using the agency’s electronic visit verification program where applicable.
• Participate in the agency’s care coordination process, reporting patient needs and changes in condition, attending case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management, and case or community managers.
• Work in any home setting regardless of the client’s age, race, creed, color, sex, disability, or financial condition.

• Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
• Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation.

• Current CPR certification or ability to obtain certification within 90 days of hire.
• Ability to work flexible hours as needed to meet client needs.
• Ability to manage multiple tasks simultaneously.
• Capability to work independently.
• Strong communication, writing, and organizational skills.

Hourly pay ranges from $20.38 to $36.44 per hour for full‑time employment. The role includes a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and a 401(k) contribution, all subject to eligibility requirements.
