Apply the nursing process to plan and implement patient care, including education and continuum of care planning. Communicate patient information to the healthcare team, collaborate on establishing patient goals, and evaluate progress to ensure effective outcomes.

• Collaborate in establishing the plan of care, coordinate and implement care delivery while anticipating patient needs across the continuum.
• Analyze comprehensive patient information to determine nursing diagnoses.
• Utilize the nursing process to evaluate patient progress toward goals and apply appropriate interventions.
• Employ strategies to promote health and maintain a safe, healthy environment.
• Communicate pertinent patient/family information to each member of the healthcare team.
• Participate in learning activities to advance professional growth in accordance with Board of Nursing standards.

• Evidence‑Based Practice: search for evidence to answer clinical questions under guidance of others.
• Care Planning: anticipate resource needs, set priorities, schedule activities, and organize work within direct responsibilities.

  Education:
  
  provide patient education in moderate‑difficulty practical applications.
• Nursing Patient Assessment & Evaluation: assess and evaluate patients in moderate‑difficulty practical applications, conduct interviews, perform physical examinations, and gather pertinent data for referral or treatment.

• Licensure:
  Registered Nurse (LIC) or other approved nursing license.
• Experience:
  
  1 year of relevant work experience.
• Education:
  
  Graduate of an approved discipline‑specific program.
• Availability:
  Minimum of one shift per month, Monday‑Friday 8:00‑16:30.
