Independent Support Coordinator

** MUST LIVE IN ONE OF THE FOLLOWING COUNTIES:
Montgomery, Robertson, Stewart, Sumner Zip Codes 37075 & 37188, Hendersonville, and White House.**

Tennessee Community Services Agency is seeking a Case Manager to provide case management services for individuals with developmental and intellectual disabilities enrolled in the Katie Beckett Part B Program.

• Conduct meetings with individuals, families, and other members of the support team.
• Develop and write Individual Support Plans (ISPs) based on each individual's needs and goals.
• Submit support plans to the Department of Disability and Aging (DDA) to request necessary services and supports.
• Complete monthly monitoring of authorized services to ensure individuals are receiving appropriate support.
• Maintain accurate and timely case documentation and complete all required paperwork.
• Communicate and coordinate with individuals, families, providers, and other involved parties.

• Bachelor’s degree required; preferred fields include Social Work, Psychology, Sociology, or a related field
  .
• One to two years of related work experience preferred.
• ISC experience and/or certification is preferred.
• Knowledge or experience with DDA and/or the Katie Beckett Program is a plus.
• Strong organizational and time-management skills with the ability to work independently, manage multiple cases, and meet required deadlines.
• Excellent written and verbal communication and documentation skills.
• A valid driver’s license is required.
• Must have reliable transportation and current automobile insurance.
• Must successfully complete all required background checks and authorize the release of applicable investigative and criminal records obtained through the Federal Bureau of Investigation (FBI) and Tennessee Bureau of Investigation (TBI) to Tennessee Community Services Agency.

• Travel is required for this position. The position serves individuals across a large geographical area. However, the employee will not be required to travel to every location within the assigned service area each month
  . Travel will vary based on caseload, scheduled meetings, monitoring requirements, and the needs of the individuals served.
