Per Diem Speech Language Pathologists (SLPs)

Ten Broeck Center for Rehabilitation and Nursing is seeking Per Diem Speech Language Pathologists (SLPs) for weekday and weekend shifts.

• Evaluates, diagnoses, and treats residents with speech, language, voice, cognitive-communication, and swallowing disorders.
• Develops and implements individualized treatment plans to improve functional independence and quality of life.
• Collaborates with interdisciplinary team members to ensure coordinated resident care.
• Ensures compliance with federal, state, and facility regulations.

Clinical Assessment & Treatment

• Conduct comprehensive speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans with measurable goals.
• Provide skilled therapy services in accordance with physician orders and plan of care.
• Perform bedside swallow evaluations and recommend appropriate diet modifications.
• Implement evidence-based interventions for dysphagia, aphasia, dysarthria, cognitive impairment, and voice disorders.
• Monitor resident progress and adjust treatment plans as needed.

Documentation & Compliance

• Complete evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in a timely manner.
• Ensure documentation supports medical necessity and skilled services.
• Comply with CMS, Medicare Part A and Part B, Medicaid, and insurance guidelines.
• Participate in audits and survey readiness activities.

Care Coordination

• Collaborate with physicians, nursing staff, dietitians, occupational therapists, physical therapists, and social services.
• Participate in interdisciplinary team (IDT) meetings and care plan conferences.
• Educate residents, families, and caregivers on treatment plans and home exercise programs.
• Provide recommendations for adaptive equipment and communication strategies.

Quality & Safety

• Ensure safe swallowing practices and aspiration precautions.
• Train staff on proper feeding techniques and swallowing safety.
• Contribute to quality assurance and performance improvement initiatives.
• Maintain infection control and safety standards.

• Master's degree in Speech-Language Pathology.
• Current, active state licensure as a Speech-Language Pathologist.
• Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association preferred.
• Experience in long-term care or skilled nursing preferred.
• Current CPR certification (if required by facility).

• Strong knowledge of dysphagia management and cognitive-communication therapy.
• Excellent clinical assessment and problem-solving skills.
• Effective verbal and written communication abilities.
• Ability to work independently and as part of an interdisciplinary team.
• Strong organizational and time-management skills.
