Job Opportunity
We are looking for a motivated and empathetic Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing clinical team. We specialize in delivering high-quality behavioral health services to individuals residing in nursing homes throughout New York State. This position is ideal for a practitioner who is enthusiastic about geriatric mental health and dedicated to supporting the well-being of older adults.
Role Overview
- Perform in-depth psychiatric evaluations and ongoing assessments for residents in skilled nursing facilities.
- Formulate tailored treatment plans that align with each patient's diagnosis and clinical needs.
- Manage psychiatric medications, including initiation, monitoring, and adjustments as appropriate.
- Partner with physicians, psychologists, social workers, and nursing staff to coordinate comprehensive patient care.
- Provide therapeutic support, counseling, and guidance to both patients and their families.
- Educate patients and caregivers on mental health conditions, treatment options, medication use, and coping strategies.
- Document all clinical encounters accurately and ensure compliance with state and organizational standards.
- Take part in team discussions, case reviews, and initiatives focused on improving clinical outcomes and care delivery.
Qualifications
- Active New York Nurse Practitioner license with certification as a PMHNP.
- Ability to manage an independent caseload with professionalism and sound clinical judgment.
- Strong assessment and diagnostic abilities; solid understanding of psychopharmacology and psychiatric interventions.
- Outstanding communication skills, with the ability to build trust and collaborate effectively with patients, families, and interdisciplinary teams.
- Comfortable using electronic health records (EHRs) and general computer applications.
- A compassionate, patient-first approach to psychiatric care.
