Responsibilities- Provide comprehensive care to clients in their homes.
- Conduct thorough patient assessments and develop individualized care plans.
- Collaborate with family members and healthcare professionals to ensure continuity of care.
- Educate clients and families on health management and wellness strategies.
- Maintain accurate medical records and documentation.
- Ensure compliance with all health regulations and standards.
- Current valid driver's license
- Strong assessment, clinical, and communication skills.
- Ability to work independently and manage time effectively.
- Knowledge of geriatric care practices is a plus.
- Familiarity with electronic medical records (EMR) systems.
