Summary:
The Supervisor of Case Management oversees the Care Management department teams and reports to the Care Management Director. This role builds strong working relationships with clinicians and staff to enhance patient outcomes and boost departmental measures related to length of stay and readmission reduction. The supervisor also identifies and solves obstacles to care coordination and workflow efficiency. Accountable for achieving productivity and quality targets.
Responsibilities:
1. Supervises and coordinates care management services, establishes efficient workflows and addresses barriers to implementation.
2. Establishes an environment of patient advocacy, supporting shared decision making and the use of decision aids.
3. Utilizes data and resources to identify at risk patients, provides education and removes barriers to care plan compliance. Monitors outcomes and works collaboratively with the care team to optimize the patient's care plan.
4. Facilitates onboarding, training, and continued education of care management staff on a variety of topics related to Care Management workflows and activities.
5. Audits for appropriate documentation in the patient's electronic medical record, monitors compliance and provides feedback and training.
6. Collaborates with post-acute providers to build relationships and facilitate appropriate and timely patient discharges.
7. Actively participates in meetings, problem solving, goal setting, performance improvement and patient satisfaction initiatives. Ensures compliance with organizational policies, procedures and evidenced based guidelines.
8. Participates in annual performance evaluation to support employee performance feedback.
9. Manages Care Management associate timekeeping, scheduling, and assignment activities to ensure appropriate coverage and payroll compliance.

* Bachelor's Degree Required.
Bachelor's Degree in a clinical or nursing related field preferred.
Master's Degree in Social Work and is LCSW preferred.
Master's degree in health care related preferred.
** Licensure/Certification Requirements:*
* Licensed to practice as a Registered Nurse or LCSW in the State of North Carolina preferred. Prefer CCM or ACM certification.
** Professional Experience Requirements:*
* Three years of broad clinical experience in the acute care setting caring for patients with complex medical conditions required. Experience in case management, utilization review, quality management preferred.
