Description

Summary- The Case Manager coordinates discharge planning, patient teaching and other associated activities for successful discharge to community/home re‑entry or to nursing facilities. In inpatient rehabilitation, medical case management involves coordinating a patient’s care, ensuring access to resources, and supporting their recovery journey. They complete, monitor and assure compliance with federal, state and third‑party insurance guidelines, work with the healthcare team to develop and implement a rehabilitation plan, monitor the patient’s progress, and assist with discharge planning.
Case managers also advocate for the patient’s needs and ensure quality, cost‑effective care.
- Build and maintain professional relationships with patients/families, physicians, community agencies, business associates and staff. Project a positive image of the department and hospital while being helpful, courteous and friendly.
- EDUCATION
- Graduate from an accredited school of nursing or medical social work. A baccalaureate degree in human services is accepted. Obtaining case manager certification within 5 years is encouraged.
- EXPERIENCE
- Two‑three years of experience in a hospital, nursing home or community agency.
- LICENSURE/REGISTRATION/CERTIFICATION
- Certification by American Board of QA/UR or Inter Qual, certification in social work, licensed in N.C. as an RN.
- OTHER SKILLS AND QUALIFICATIONS
- Knowledge of Inter Qual ISD‑A criteria. Good interpersonal and communication skills. Understanding of patient care needs across developmental stages for various age groups. Knowledge of state and federal regulations regarding admission/reimbursement of Medicare/Medicaid and other payor sources. Proficiency in reading and mathematics. Ability to work autonomously.
- WORK ENVIRONMENT
- Exposure to airborne and blood borne pathogens, exposure to electrical hazards (equipment use).
- PHYSICAL REQUIREMENTS
- Standing, walking, sitting, reaching, talking, seeing, hearing, tasting, smelling, carrying, pushing, pulling, lifting and moving.
