Gentiva Hospice is seeking a dedicated Registered Nurse Case Manager to deliver hospice and palliative nursing care in patients' homes or other settings. You will conduct comprehensive assessments, implement and adjust plans of care, and collaborate with an interdisciplinary team to ensure compassionate, person-centered support for patients and families.
You will educate families, document care with accuracy, and participate in on-call coverage and quality improvement efforts.
