We have an opportunity for a 1.0 FTE primary care Advanced Practice Provider at our regional medical center in Kirkland, WA. • Integrative primary care model: VMFH has an innovative approach, with embedded pharmacists, social workers and psychiatry providers all part of the primary care team to deliver coordinated care and address social/emotional/behavioral/environmental needs.

Nurse Practitioner

• Washington State Registered Nurse License and an Advanced Registered Nurse Practitioner License are required.
• Drug Enforcement Agency (DEA) and National Provider Identifier (NPI) numbers may be required depending on the position, as well as prescriptive authority.

Physician Assistant–Certified

• Certification as a Physician Assistant with prescriptive authority in Washington State.
• DEA and NPI numbers may be required depending on the position, as well as prescriptive authority.

$136,600 – $167,000 per year
