Qualifications & Skills

• Active licensure and certification (e.g., ANCC, AANP, NCCPA) in relevant fields.
• Unrestricted state license and DEA prescriptive authority.
• Reliable transportation and ability to navigate varied environments.
• Preferred: 1+ years clinical experience, geriatric or home health background, proficiency with EMRs, and experience managing complex patient needs.

Key Duties

• Conduct home assessments including medical history, vital signs, and physical exams.
• Manage acute episodes and chronic condition exacerbations.
• Provide patient and caregiver education and counseling.
• Document thoroughly and communicate effectively with the care team.
• Participate in advanced care planning and evidence-based screenings.
