Qualifications & Skills
- Active licensure and certification (e.g., ANCC, AANP, NCCPA) in relevant fields.
- Unrestricted state license and DEA prescriptive authority.
- Reliable transportation and ability to navigate varied environments.
- Preferred: 1+ years clinical experience, geriatric or home health background, proficiency with EMRs, and experience managing complex patient needs.
Key Duties
- Conduct home assessments including medical history, vital signs, and physical exams.
- Manage acute episodes and chronic condition exacerbations.
- Provide patient and caregiver education and counseling.
- Document thoroughly and communicate effectively with the care team.
- Participate in advanced care planning and evidence-based screenings.
