We’re looking for a Clinical Case Manager (CCM) to provide comprehensive behavioral health support to resident populations. In this role, you’ll deliver individual and group therapy, manage a caseload of 10‑15 residents, coordinate clinical services, and provide crisis follow‑up to lessen behavioral health inequities and improve wellbeing.
You’ll work as part of the Resident Wellbeing Care Team—collaborating with peer specialists, case managers, and community resource navigators—to ensure seamless, trauma‑informed care within Plymouth Housing’s supportive housing model.
PRIMARY RESPONSIBILITIES- Collaborate with residents to assess behavioral health needs and develop individualized service plans.
- Facilitate access and engagement with behavioral health, substance use, and medical treatment services.
- Provide individual and group therapy, support linkage to care programs, and conduct follow‑up after crises.
- Connect residents with internal and external clinical resources as appropriate.
- Partner with Resident Wellbeing Case Managers to identify and overcome barriers to accessing care.
- Provide advocacy and assistance helping residents navigate healthcare systems and providers.
- Engage individuals using a trauma‑informed, culturally responsive approach while maintaining professional boundaries.
- Work closely with Resident Wellbeing Care Team members, including peer specialists, case managers, and community resource navigators, to coordinate services.
- Participate in multidisciplinary team meetings and contribute to behavioral health service planning.
- Maintain accurate and timely documentation of resident interactions, interventions, and service plans.
- Builds trust and works respectfully with people from many backgrounds and lived experiences, contributing to safe, welcoming housing communities where residents experience dignity, choice, and belonging.
- Uses self‑awareness and humility to communicate effectively across differences, adapt approach, and treat people with fairness and respect.
- Strong clinical assessment, case management, and crisis intervention skills.
- Skilled in harm reduction, motivational interviewing, trauma‑informed care, and housing‑first principles.
- Ability to communicate clearly and effectively, both orally and in writing, with individuals from diverse backgrounds.
- Excellent organizational, time management, and problem‑solving skills with the ability to manage multiple priorities.
- Strong interpersonal skills; able to build trust and maintain professional boundaries with residents and colleagues.
- Proficient in Microsoft Office Suite and comfortable working with databases or cloud‑based systems (e.g., Salesforce).
