We are seeking a talented and experienced Registered Nurse to join our assessment team as a Minimum Data Set (MDS) Coordinator. As a key member of our care team, you will be responsible for conducting admission/skilled assessments to develop individualized care plans, ensuring our residents receive the highest level of personalized attention and care.

• Conduct thorough assessments of residents, including MDS assessments, to identify care needs and develop individualized care plans.
• Collaborate with interdisciplinary team members to ensure effective care delivery and coordination.
• Monitor and evaluate resident outcomes, adjusting care plans as necessary to ensure optimal health and well‑being.
• Maintain accurate and timely records of resident assessments, care plans, and outcomes.
• Participate in quality improvement initiatives to enhance the overall quality of care and services.
• Participate in on‑call rotation with weekends as needed.

• Current Registered Nurse (RN) licensure in the State of Missouri, or current multistate license.
• Experience as an MDS RN, with a minimum of 2 years in a similar role.
• Strong assessment and documentation skills.
• Effective communication and interpersonal skills.
• Ability to work collaboratively as part of a multidisciplinary team.
• Strong problem‑solving and critical‑thinking skills.
