Phoenix Home Care and Hospice in Kirkwood, MO is seeking an RN Case Manager for a full-time, Monday–Friday role in home health. You’ll manage a caseload, supervise LPNs, and coordinate with PT/OT/SLT teams while building individualized care plans aligned with physician orders.
Expect to work with conditions such as CHF, COPD, diabetes, wounds, and medications. A real schedule with limited on-call, plus a guaranteed training period, awaits you.
