Role Overview:
Responsible for assessing residents' health, educational, and environmental needs, developing and updating individualized care plans, and ensuring safe, timely delivery of nursing interventions. Communicates effectively with the interdisciplinary team, residents, and families, documenting assessments and care activities accurately. Monitors residents' conditions, responds to changes with critical thinking, and utilizes QAPI processes for problem resolution. Provides direct resident care, including assistance with ADLs, and maintains professional knowledge of nursing practices.
Qualifications & Skills- Registered nurse with an accredited nursing education and current license in the state
- CPR/AED certification (or willing to obtain within six months)
- Critical thinking, excellent communication, time management, and organizational skills
- Physical ability to stand, walk, lift, and transfer residents weighing over 200 lbs safely
- Manual dexterity, visual acuity, and proficiency in electronic and written documentation
- Preferred experience in long-term care setting
Conduct assessments, develop and revise care plans, execute nursing interventions, communicate with team members, document all activities, respond to resident changes, and participate in ongoing training.
