Comforting, Loving & Helping Hands Home Care is seeking an experienced Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout the Kissimmee, FL area. This role includes initial evaluations
, Plan of Care development
, and ongoing treatment visits
, with compensation structured per visit or hourly depending on the case and service provided

This position is ideal for therapists who are confident working independently, experienced in home health, and skilled in completing thorough clinical documentation that supports patient progress and care planning.
Who This Role Is Best ForThis role is well‑suited for Physical Therapists who:
- Have a minimum of two (2) years of clinical experience
- Have experience in home health or community‑based therapy
- Are comfortable completing comprehensive evaluations
- Understand the importance of documentation in Plan of Care development
- Work independently while maintaining high clinical standards
One‑on‑One Patient Care
Deliver focused, individualized therapy in patient homes without the pace of outpatient or facility‑based settings.
Flexible Compensation Structure
- Compensation may be per visit or hourly
, depending on the case - Evaluation visits and routine treatment visits are compensated differently based on scope and responsibility
Flexible Scheduling
- PRN, per‑diem, or flexible caseloads
- Visit schedules coordinated based on therapist availability and patient needs
- Perform initial physical therapy evaluations
- Develop and update individualized Plans of Care
- Provide skilled therapy treatment visits as ordered
- Assess patient mobility, strength, balance, and functional ability
- Educate patients and caregivers on exercises, safety, and mobility strategies
- Communicate effectively with physicians and interdisciplinary team members
- Complete accurate, timely, and defensible clinical documentation
PT documentation is a critical component of patient care and is used to:
- Establish and update the Plan of Care
- Support medical necessity for skilled therapy services
- Track patient progress and outcomes
- Meet payer and regulatory documentation requirements
Strong attention to detail and timely completion of notes is required.
Requirements- License: Active Florida Physical Therapist (PT) license
- Experience: Minimum two (2) years of clinical experience

Ability to complete detailed, accurate clinical notes - Compliance: CPR certification and Level II background screening
- Transportation: Ability to travel to patient homes
