Overview
Director of Case Management — Kissimmee, Florida
The Director of Case Management provides strategic leadership and operational oversight for the Case Management department. This role drives Care Coordination, Care Transitions, and compliance while aligning initiatives with the Divisional Director of Case Management. The Director works closely with leadership to implement corporate Case Management strategies and collaborates regularly with the CMO, CNO and CFO to ensure alignment and bidirectional communication.
Qualifications- Registered Nurse (RN) required
- BSN required
- MSN preferred
- Director of Case Management experience in an Acute Care Jospital setting required
- Progressive Case Management leadership responsibility
- ACM or CCM certification preferred
- Provides leadership and direction to Case Management managers, team leads, RN Case Managers, Social Workers, and assistants to ensure consistent clinical, service, and financial outcomes
- Implement corporate Case Management strategies and best practices to optimize throughput, reduce length of stay, and improve discharge planning
- Oversees daily operations, including care coordination, care transitions, and compliance with regulatory guidelines (CMS, Joint Commission, IMM/MOON)
- Develops and monitors performance improvement initiatives, analyzes metrics (LOS, avoidable days, denials), and creates mitigation plans
- Collaborate with physicians, nursing, and multidisciplinary teams to eliminate barriers, facilitate timely progression, and align care with patient needs
- Escalates unresolved status conflicts or financial/operational concerns to Physician Advisor(s) as appropriate
- Ensures regulatory compliance, survey readiness, and adherence to evidence-based standards
- Manages department budget, staffing, recruitment, retention, and professional development
- Fosters physician alignment, interdisciplinary cooperation, and a high-performance culture
- Utilizes data analytics to drive decisions, identify variances, and implement process improvements
