We’re expanding our team in Brevard County and looking for skilled nurses who take pride in quality care and teamwork.

We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide direct patient care in the home setting under the supervision of a Registered Nurse (RN). The LPN will deliver skilled nursing services, assist in implementing patient care plans, and help promote positive clinical outcomes for our home health patients.

• Provide skilled nursing care to home health patients as directed by the supervising RN and physician orders
• Assist with OASIS Start of Care, Resumption, Recertification, and Discharge visits as needed
• Administer medications and treatments accurately and safely
• Monitor and document patient condition, progress, and response to care
• Communicate any changes in patient status to the RN Case Manager and physician promptly
• Educate patients and families on care techniques, medications, and home safety
• Maintain timely, accurate documentation in compliance with Medicare and agency standards
• Collaborate with the interdisciplinary team to ensure quality and continuity of care

• Active Florida LPN license (required)
• Minimum 1 year of Home Health experience (Medicare-certified agency)
• Current BLS certification
• Strong clinical skills, attention to detail, and communication abilities
• Reliable transportation, valid driver’s license, and current auto insurance
