Position Overview
Join our collaborative, multidisciplinary team at Center Well Senior Primary Care. As a Nurse Practitioner or Physician Assistant, you will be part of a team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more, dedicated to delivering personalized, high‑quality care to seniors.
Primary ResponsibilitiesYou will work on site at our Center Well Poinciana location, reporting to the associate medical director. As the provider of the day, you will see walk‑in and some hospital follow‑up patients and referrals.

- Master’s degree as a nurse practitioner or completion of a physician assistant program with board certification.
- Active, unrestricted NP or PA license in the state of Florida.
- Two years of experience in value‑based care, geriatric or primary care, managing adults aged 65+ with chronic conditions.
- Proficiency using an EMR.
- If selected, you will be required to be screened for TB.
- Fluent in Spanish and English.
- Urgent care or emergency medicine experience.
- Experience in Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain a DEA license and/or Medicaid and Medicare numbers.
- Familiarity with HEDIS quality indicators and Medicare guidelines.

Scheduled Weekly Hours40 hours per week.
Pay Range$123,800 - $170,300 per year (base pay). The range may vary based on location and individual qualifications.
