Family Nurse Practitioner – Community‑Focused Primary Care
Location: Klamath Falls, Oregon
Job Type: Full‑Time

We’re seeking a dedicated Family Nurse Practitioner (FNP) to provide primary care and health promotion services in a collaborative, patient‑centered environment. Responsibilities- Provide primary care services including well‑child visits, school physicals, immunizations, and chronic disease management
- Conduct health education and promotion for individuals and groups
- Administer or prescribe treatments and medications as appropriate
- Order, perform, and interpret diagnostic tests and lab work
- Evaluate and treat minor illnesses and injuries
- Maintain accurate and timely documentation in medical records
- Refer patients for specialty services when needed
- Perform procedures within scope of competency
- Uphold strict patient confidentiality and clinic protocols
- Collaborate with medical leadership on additional services as needed
- Serve as a key Family Nurse Practitioner (FNP) resource for patients navigating chronic conditions, preventive care, and community health services.
- Completion of MSN and accredited Family Nurse Practitioner program
- Current Oregon license to practice medicine, DEA license, and CPR certification
- Current BLS certificate
- Strong communication and teamwork skills
- Ability to work independently and collaboratively in a fast‑paced setting
- Commitment to confidentiality and culturally responsive care
- Willingness to work occasional evenings and weekends
- Experience in FQHC or underserved settings preferred
