Position SummaryThe Licensed Practical Nurse (LPN) plays a key role in delivering high‑quality, in‑scope nursing care as part of an interdisciplinary medical team. This position supports patient assessments, clinical procedures, education, and care coordination to ensure a positive and safe patient experience.

- Assist physicians and RNs with patient examinations and treatments.
- Administer medications and injections as prescribed.
- Obtain and monitor vital signs, complete medication reconciliation, and document clinical updates.
- Perform routine procedures such as lab specimen collection and EKGs.
- Conduct nurse visits, including wound care, IV therapy, vaccinations, BP checks, patient education, oxygen walk‑tests, EKGs, and catheter care.
- Provide patient and family education on conditions, treatments, and preventive care.
- Conduct health screenings, immunizations, and focused assessments.
- Support scheduling of appointments, procedures, and follow‑up care.
- Coordinate referrals to outside providers and facilities.
- Maintain accurate and timely medical documentation.
- Collaborate with the interdisciplinary care team to support comprehensive patient care.
- Participate in training, quality improvement, and professional development activities.
- Foster a safe, welcoming, and patient‑centered clinic environment.
- Uphold confidentiality, professionalism, and ethical standards.
- Completion of an accredited LPN program.
- High School Diploma or equivalent.
- Eligible to provide services under federal healthcare programs (not excluded by OIG/GSA).
- Proficiency with electronic health records and basic computer applications.
- Strong organizational, multitasking, and communication skills.
- Ability to work independently and collaboratively.
- Proficiency in written and spoken English.
- Ability to meet KHP immunization requirements.
- Ability to pass a background check and drug screen.
- BLS certification (or ability to obtain within 180 days of hire).
- Bilingual in English and Spanish.

- Ability to open/close prescription vials and read for extended periods
- Well‑lighted, ventilated clinical environment
