Licensed Practical Nurse (LPN) – Nurse Clinic (Outpatient)
The Licensed Practical Nurse (LPN) provides direct patient care in a nurse clinic within an outpatient setting. The LPN performs delegated nursing tasks and protocol-based care under the supervision of a Registered Nurse (RN) or licensed provider, in accordance with Oregon State Board of Nursing guidelines. Primary responsibilities include lab draws, immunizations, wound care, and other task-oriented clinical services.
Major duties and responsibilities include:
- Perform patient intake, including vital signs, allergies, and medication reconciliation
- Conduct venipuncture and capillary blood collection for ordered laboratory testing
- Process, label, and prepare specimens for transport in accordance with lab protocols
- Administer medications and injections, including vaccines, per provider orders and approved standing orders
- Monitor patients for immediate adverse reactions following medication or vaccine administration
- Provide immunizations in accordance with clinic protocols, standing orders, and state guidelines
- Perform wound care, including dressing changes, basic wound assessment, and documentation of healing progress
- Conduct ear lavage, simple treatments, and other approved clinical procedures
- Perform point-of-care testing (e.g., glucose, A1c, urine testing) per protocol
- Reinforce provider-directed patient education related to treatments, medications, and follow-up care
- Practice under the direction of an RN or licensed independent provider as required by the Oregon State Board of Nursing
- Perform only those duties that are within LPN scope and for which competency has been validated
- Follow established standing orders, clinical protocols, and delegated tasks
- Report patient status, abnormal findings, and concerns to the supervising RN or provider promptly
- Do not perform independent nursing assessments, triage, or care planning beyond LPN scope
- Accurately document all care, procedures, and patient interactions in the electronic health record (EHR)
- Maintain compliance with infection control standards, including proper handling of sharps and biohazard materials
- Ensure patient confidentiality in accordance with HIPAA regulations
- Follow medication administration safety standards, including the "Five Rights"
- Maintain clean, stocked, and organized nurse clinic and treatment areas
- Ensure proper storage and handling of vaccines, including cold chain management
- Monitor and restock clinical supplies as needed
- Participate in competency validation and ongoing skills training
- Like all employees of the Klamath Tribes, the incumbent will be called upon to accomplish other tasks that may not be directly related to this position, but are integral to the Klamath Tribes' broader functions, including but not limited to, assisting during Tribal sponsored cultural, traditional, or community events that enable the successful operation of programs and practices of The Klamath Tribes as aligned with The Klamath Tribes' Mission Statement.
Some of these tasks may be scheduled outside of regular work hours, if necessary.
Knowledge, skills, abilities:
- Ability to maintain strict confidentiality of medical records and adhere to the standards for health record-keeping, HIPAA and Privacy Act requirements.
- Proficiency in venipuncture and specimen handling
- Competence in vaccine administration and injection techniques
- Knowledge of basic wound care and infection prevention practices
- Understanding of LPN scope of practice and delegation requirements in Oregon
- Ability to follow established protocols and standing orders with accuracy
- Strong documentation and organizational skills
- Effective communication with patients and healthcare team members
Qualifications, experience, education:
- REQUIRED to possess a High School Diploma or Equivalent. (Must submit copy of diploma or transcripts with application.)
- REQUIRED Current Licensed Practical Nurse (LPN) license in the state of Oregon
- REQUIRED Current Basic Life Support (BLS) certification
- REQUIRED Graduate of an accredited practical nursing program
